I, the undersigned, in accordance with the legislation on the protection of personal data, hereby give my consent to the personal data controller, individual entrepreneur Oleg Botvinov (hereinafter — the Coach), to process my personal data, namely: surname, name, patronymic (if available), contact phone numbers, email addresses, start and end dates of coaching, the number of hours (free and paid) within coaching sessions, and bank details.
I give my consent to the Coach to perform the following actions with my personal data: to enter the data into the client / coaching-session spreadsheet and store this spreadsheet on the Coach's electronic devices and in cloud storage owned by the Coach (including SaaS), and to use this information to record and systematise the Coach's work.
I also allow the Coach to transfer my personal data to the International Coaching Federation if such a request is received from the Federation. I allow the ICF to contact me using the contact data I have provided in order to confirm the fact of my work with the Coach.
If the GDPR rules apply, this consent shall be deemed consent to the processing of my personal data by the Coach under the GDPR rules.
This consent for personal data processing is valid indefinitely and may be withdrawn by me at any time by sending a written notice to the Coach.
